What are some insider tips for filling out the Webinar Request Form?
It sounds basic, but please read through the form before you start filling it out; gathering all of the appropriate information before you begin will make the process much easier. Also, be as detailed as possible in your responses.
Who needs to fill-out/complete the Webinar Request Form?
The project initiator or the webinar presenter should complete the project request questionnaire. All required questions must be answered in order to submit the form.
Once the form is submitted, what happens next?
Once you submit the request form, it will be sent via email to the marketing and IT departments. IT will set up the webinar and send out links to the event. Marketing will create the flyer using the links from IT and will send out the flyer for approval. The project initiator must approve the flyer before we being to advertise the event.
When does the Webinar Request Form need to be completed?
- Webinars that do not offer CEUs should be submitted 6-weeks prior to webinar presentation date.
- Webinars that offer BACB CEUs should be submitted 6-weeks prior to webinar presentation date.
- Webinars that offer ASHA CEUs should be submitted 8-weeks prior to webinar presentation date.
Meeting the deadlines above will give us enough time to create the flyer and advertise the webinar.
What are the requirements for ASHA CEUs to be offered at a webinar?
If you plan to offer ASHA CEUs at your next webinar, Mary Bainor, ASHA CE Administrator for Eden II, must be notified prior to the submission of the webinar request form. Mary will register the course with ASHA and will provide you with the total number of CEUs that can be offered during this webinar (the number of credits must be included in the request form). ASHA must review and approve the webinar event flyer that is created which must also contain the ASHA brand block, financial disclosures, and non-financial disclosures.
*Failure to adhere to the ASHA guidelines will result in a penalty and webinar attendees will not receive CEUs.
How do I provide financial and non-financial disclosure information on the webinar request form?
Course planners and instructional personnel must disclose relevant financial and nonfinancial relationships. Financial relationships are those relationships in which the individual benefits by receiving a salary. Nonfinancial relationships are those relationships—including personal, professional, political, institutional, religious, or other—that might bias an individual.
What are some examples of financial and non-financial disclosure statements?
Financial Disclosure Examples:
- Jane Doe is employed by Eden II Programs where she receives a salary.
- There are no relevant financial relationships to report.
Non-Financial Disclosure Examples:
- Jane Doe is the mother of a child on the Autism Spectrum.
- There are no relevant non-financial relationships to report.
